Applying for Accommodations

Students will need to register with the Student Accessibility Services (previously Office for Students with Disabilities – OSD) in order to receive disability accommodations. Be sure to register early with us before classes start as it may take several weeks to process your registration and to make all necessary accommodations.

Please note that all communication and documentation is strictly confidential between the student and Student Accessibility Services. No information, except as provided by law, will be released to anyone, including parents, without student’s written consent.

Step 1: Contact Student Accessibility Services

Students who have not requested accommodations from the Student Accessibility Services before should contact the office at (352) 365-3589 to discuss potential services and documentation needed to determine eligibility for services.

Step 2: Provide documentation

Review the documentation guidelines for the type of disability the student is disclosing. Provide the documentation to the Student Accessibility Services for review. If the documentation supports your eligibility, then an appointment is scheduled to discuss what accommodations you need.

Step 3: Accommodations appointment

If the documentation supports your eligibility, then the student will be contacted for an intake appointment to discuss accommodations.

Accessing Services

After the intake appointment and being registered for classes, the student will need to request Instructor Notification Letter (INL) forms for each class for which accommodations are required.
An INL request must be submitted each semester to receive accommodations.

While the INL informs your instructor that you are entitled to accommodations, it is still the student’s responsibility to communicate with the instructor about specific needs. Services or accommodations can only be provided for a student who provides proper documentation

Students requesting accommodations have certain rights and responsibilities throughout the process.

Confidentiality

All documentation provided to the Student Accessibility Services regarding a student’s disability will be handled in a discrete and confidential manner. No information, except as provided by law, will be released to anyone, including parents, without the student’s expressed written consent.
